Brain Basics Form

A limited number of caregivers will be eligible to receive a subsidized course rate of $50 per person. This special rate is made possible through the generosity of our sponsors. If you are interested in attending and meet the criteria below please fill in the Subsidy Form and return to the Brain Injury Association of Windsor/Essex County. If we have more qualified applicants than spaces allowable a random draw will take place to fill these spaces.
