Ontario Trillium Foundation

The Ontario Trillium Foundation’s mission is to help build healthy and vibrant communities by strengthening the capacity of the voluntary sector through investments in community-based initiatives. This is reflected in our granting priorities and our grants that enable organizations to build on their strengths and develop new ideas.

Ministry of Community and Social Services

The goal of the Ministry of Community and Social Services is to build thriving and inclusive communities sustained by the economic and civic contributions of all Ontarians. The ministry’s mission is to build resilience and remove obstacles that impede opportunity and participation in community life. It does this through policy development and program support for a broad range of services aimed at supporting Ontario’s most vulnerable citizens.

Assistive Devices Program (Ministry of Health)

The objective of the Assistive Devices Program (ADP) is to provide consumer centered support and funding to Ontario residents who have long-term physical disabilities and to provide access to personalized assistive devices appropriate for the individual’s basic needs.
Devices covered by the program are intended to enable people with physical disabilities to increase their independence through access to assistive devices responsive to their individual needs.

Windsor

Community Care Access Centre (CCAC)

Your Community Care Access Centre (CCAC) connects you with the care you need, at home and in your community:
• They can help you stay in your own home longer by providing Care in Your Home and by coordinating Care in Your Community, including specialized support services
• They can provide you with information about Long-Term Care Options if it becomes too difficult for you to live independently at home

Hotel-Dieu Grace Hospital

Hôtel-Dieu Grace Hospital is this region’s premier tertiary acute care hospital. Combining a century-old tradition of compassionate care with cutting-edge diagnostic imaging technology, HDGH leads the provision of advanced care in the areas of complex trauma, renal dialysis, acute mental health, cardiac care, stroke and neurosurgical, and the broad foundation of medical and surgical services required to support these areas of specialization.

Windsor Regional Hospital Acquired Brain Injury Team

This is a community-based service for individuals who have sustained a brain injury. The Acquired Brain Injury (A.B.I.) Consultation Team is committed to facilitating community reintegration and enhancing the psychosocial functioning of individuals with brain injury. The team collaborates with other professionals and family members to promote the best possible care for the client in his or her own environment.

Windsor Family Team

The Windsor Family Health Team is a non-profit corporation offering a community governed family health team. Patient enrollment is required to access these services. The family health team commits to offering quality primary health care in a collaborate setting that is aligned with our vision, mission, goals and values as follows:
The citizens of Windsor will have access to the highest quality primary health care in a timely manner from an integrated team of health care providers in a welcoming, caring environment.
The Windsor Family Health Team will enable improved access to high quality, holistic primary health care in a multidisciplinary setting.

Windsor-Essex County Health Unit

The Windsor-Essex County Health Unit, in partnership with other agencies, and health care providers, seeks to enable all Windsor and Essex County residents to be as healthy as possible.

London

Parkwood Hospital

Parkwood Hospital is a London city-wide Rehabilitation Institute servicing those with including spinal cord, brain injury, stroke, amputee and musculoskeletal programs. Other programs include: Complex Care, Palliative Care, Veteran’s Care and Specialized Geriatric Services.
